Our Floating Support Services provide flexible, tailored support depending on a person's specific needs and situation.
Floating Support Teams
Our specialist Floating Support Teams provide one to one support to people accessing these services within their own homes, each person will have their own individual support plan to work towards their own individual goals.
We cover a wide geographical area, with teams spread across South and West Wales, including Newport, Cardiff & The Vale, Merthyr Tydfil, Caerphilly, Monmouthshire, Torfaen, Blaenau Gwent, Pembrokeshire, Carmarthenshire, Ceredigion and Powys.
Mental Health Support Services
In conjunction with Cardiff Mental Health Team, we support people aged 18+ with enduring mental health difficulties.
We also cover the Ceredigion area, with our specialist team providing tenancy support to individuals experiencing or recovering from poor mental health.
Housing First
Our Housing First team works works tirelessly to help people experiencing homelessness to move into temporary and permanent accommodation.They provide support when people need it most - and Scott's story illustrates how important it really is...
Supporting Young People
We support young people aged 16-25 to successfully move on from supported accommodation or care. This support is both practical to ensure tenancies are maintained and focused on the social, emotional, training and employment needs of young people. We know it is these elements that lead to successful and fulfilling lives for young people.
We provide this service in a number of areas, including Merthyr Tydfil, Caerphilly and Monmouthshire.
Preventing Homelessness
We provide housing related support to prevent homelessness and social isolation. Our role is to work with customers to help them gain the skills and confidence to sustain their tenancies, enabling them to live life their way.
We work to a set of clear principles which put the customer first, taking time to understand what matters to them so we can work on the right thing at the right time to support them.
This service is available in a number of areas including, Powys, Merthyr Tydfil, Blaenau Gwent, Bridgend, Caerphilly, Newport, Monmouthshire and Pembrokeshire.
